The life insurance application process consists of a written application, medical exam and underwriting.  It may also be necessary for the life insurance company to contact you for a brief telephone interview.  You can begin the application process on line today and obtain the insurance policy within a few weeks.

Fill in the e-mail application request form below to  request an application.  Once you submit the form, we will contact you to obtain additional information to complete your application (this will take around five minutes).  Your application will be mailed to you along with a detailed illustration of the premiums.  Review the application, complete any additional information needed and return it in the envelope provided.

Two

If a medical examination is required, you will be contacted to set up a convenient time for the exam.  The medical exam can be scheduled at your home or office and will be paid by the insurance company.

Three

Once we receive your completed application and medical exam, it will be forwarded to the insurance company you had selected for review and issuance of the policy.  Underwriting usually takes four to eight weeks.  We will inform you when the underwriting has been completed and request a check for the premium at that time.
